Complaints about pesticide use made to the MDA must be submitted in writing according to state law. Please fill in and submit the form below.
If your complaint DOES NOT involve products containing the new dicamba formulation, i.e. Engenia, Tavium, or XtendiMax, or if you are unsure, DO NOT use this form. All unknown or other pesticide complaints should be submitted using the general Pesticide and Fertilizer Misuse Complaint Form.
