The following instructions will help guide you in completing the form. This report is mandatory and must be filed by July 31st of each year even if your firm has zero “0” sales to report and/or no fees are required.

INSPECTION FEE DUE COLUMNS (Fertilizer sold to end user (grower)
This section (left side of form) is to be completed in its entirety by the person responsible for the payment of the inspection and ACRRA fees to the Minnesota Department of Agriculture (MDA).
FEE EXEMPTION COLUMNS (Fertilizer sold to licensed dealer/distributor)
This section (right side of form) is to be completed in its entirety if you are claiming an exemption (fertilizer sold to another licensed agricultural fertilizer dealer, wholesaler and/or distributor).
Add the dollar amounts from lines 1a, 2a, and 3a, enter the amount due on the “Total Fees Due” line.
Upon completion of the tonnage report, certify by completing the signature block information located on the bottom of page 1, save a copy of the report for your records, and mail the report, along with the fees due (shown on the “Total Fees Due” line) to 625 Robert Street North, St. Paul, MN 55155.
